medicine

okay..one week of medicine has passed by..i was the first one to present a case..thank god the prof who was teaching that day was nice.

the week began smoothly..but as the days passed by it became not so smooth..on opd we had class with the head unit..we had to present a case..a male patient..disoriented..fever..history of vomiting for 3 days..and hepatomegaly ..and not to forget smelled of alcohol..
oh man..never in my life i've smelt alcohol so strong that i can feel it in the back of my throat..so coming back to the discussion..since he was smelling of alcohol + disoriented + vomiting + hepatomegaly..we all thought of GERD caused by chronic alcohol drinking with alcoholic liver disease.it masked our minds from thinking otherwise.

during the discussion with prof we got screwed just because of h/o vomiting for 3 days..and the discussion went on about vomiting..and as usual everyone kept their mouths shut not knowing what to answer..but the prof wants to hear an answer from each and everyone..we answered irrelevent stuff and he told us to go back to kindergarten..heh..so much of being in final year..patient actually had enteric fever..

next day was grand rounds..the bed i was assigned to was where the rounds started..the whole unit was there..pg presented the case..of course we also had to know about the case..patient had malaria and blood shows thrombocytopenia..i've read everything about malaria..but the question i got was what was the clinical feature of thrombocytopenia..the whole unit was staring at me..only i was allowed to answer..i froze..blank..he told to look at the patient's skin..i could see nothing when i examined him..he was dark skinned..he gave so many clues...people started whispering the answer..ok..petichiae and sometimes ecchymosis..damn..kindergarten again..

the day before grand rounds was the most tiring one..we had to know all cases in detail..and if you're lucky patient will tell you..if not so lucky they'll push you away or scold you in kannada..which we don't understand and ignore..come on patients..how will you get well if you don't tell us your problem..

and i have to prepare seminar on hypothyroidism for this coming tuesday..i'm so tired..but this is the reality..this is only having 2-3 patients under your care..not the whole ward yet..how will i cope with the tiredness when i work?? i told my mom that i want to work near home so that i can come home to my mom's cooking..hehe..too tired to cook..to tired to do anything..to get my energy back i have to have my daily dose of caffeine..which has been increased to one big glass..no more cups..

btw..i cooked chicken curry today to be eaten for the whole of next week with instant chapati..just reheat and eat..simple!!
if i did'nt cook today..i'll be ordering food for the whole week and i really have no idea what else to order coz i've been ordering food for the past so many days..getting bored of it already..
