casualty-day 1

well night actually..
i had mixed feeling before starting this posting..scared, excited, thrilled and some more feelings i can't put into words..

well yesterday my duty began at 8.30pm..it was the first time for me to be working independently..meaning no  pg's were there..only me and my fellow co-intern..and wenlock hospital is a referral center..so all serious cases from all over mangalore and near by villages will come there..meaning we have to manage them..

here is how it works..ambulance come..my heart will be racing..u never know what case u'll get..and by the looks of yesterday most are accident cases..then patient will be brought in by a trolley..our job is to first assess the patient..check the vitals, ask history, check his orientation, ask if there is any pain, then call the respective pg for further management..

yesterday my first case was a road traffic accident..an old lady..both legs broken..arm is broken..but sadly she was brought dead on arrival..next a c/o fall again an old lady..she was disoriented, hypertensive patient, turned out she had massive intracranial bleed..then two wackos..two young guys came in laughing showing their abrasions, telling they fell from their bike..and they were so happy about it..hmmm..

then another RTA case..but already partly managed by a secondary centre..he was operated that same night..then another RTA under the inflence of alcohol..funny talking with people under the influence..suddenly they'll burst into rage..next sea snake bite..it makes me wonder what in the world was he doing in the sea in the middle of the night..sea snake is very poisonous..we have to use their specific anti-venom..not the one commonly available..we referred him to another hospital since our icu was full..

then another guy came with bleeding from his ear..when asked what happened he said he did'nt know..he said he was lying down and suddenly blood came..there was a big laceration in his scalp..blood from ears..hematoma under the scalp..he was shivering and feeling nauseous..we sutured him up and sent him for CT..i wonder whatever happened to him..

this was my experience the first night..interesting and weird cases come in the middle of the night..when people are away in their beds sleeping..we'll see how it goes tonight..hope not to bad..
